Description: The Award Ceiling for Year 1 is 0 (none). CDC anticipates an Approximate Total Fiscal Year Funding amount of $8,000,000 for Year 1, subject to the availability of funds.
This NOFO will improve and strengthen HIV community-based prevention, response, and linkage to support the attainment of National 95-95-95 targets and other HIV related targets in CDC-supported provinces in Zambia. The recipient(s) of this award will provide community-based HIV services, including but not limited to index tracing, tracing of patients lost to follow up (LTFU), linkage to treatment, and following mother-infant pairs. This award will also provide technical assistance (TA) through training, mentorship, advice, and direct support to staff at the Provincial, District, Facility, and Community levels to enable them to undertake and implement evidence-based community activities through collaborations in the implementation of HIV/AIDS/TB programs.
