The projected long-term outcomes of this NOFO among adults with IDD and adults with ML include:1) improved access to recommended preventive health care and other health services; 2) reduced unmet healthcare needs for mental health conditions, (e.g., depression and anxiety) and their risk factors; 3) increased healthy lifestyle behaviors and physical, mental, and emotional wellness; and 4) reduced prevalence of chronic conditions, (e.g., diabetes) and their risk factors. |
