This grant is for communities to enhance their health information technology (health IT) infrastructure and exchange capabilities. The program aims to demonstrate a future where healthcare stakeholders are meaningful users of health IT, leading to measurable improvements in healthcare quality, safety, efficiency, and population health. Approximately 15 cooperative agreements will be awarded to qualified non-profit or government entities representing geographic healthcare communities. Ideal recipients are established leaders in health IT, workflow redesign, care coordination, or quality monitoring, possessing advanced rates of electronic health record (EHR) adoption and health information exchange (HIE). Selected communities will evolve their competencies over a 36-month period, generating and sharing valuable lessons to benefit the nation.
