Description: The Award Ceiling for Year 1 is 0 (none). CDC anticipates an Approximate Total Fiscal Year Funding amount of $2,000,000 for Year 1, subject to the availability of funds.
This NOFO will support implementation and expansion of high-quality comprehensive HIV prevention and treatment services in line with the goals of achieving national epidemic control, while simultaneously strengthening the capacity of Siaya County Government Department of Health Services (DHS) to manage, sustain and own HIV service delivery. For sustainability and efficiency, HIV services will be integrated into the routine health-care delivery system, with strong linkages to the community. DHS will be responsible for the development and implementation of a HIV services ownership and oversight management plan, and health finance plan leveraging resources from the county, national government and external partners to fund HIV service delivery in six sub-counties. Expected outcomes include strengthened county-led health systems as evidenced by increased resource allocation, enhanced leadership and implementation of the HIV program by the DHS and County government, improved coverage and quality of HIV services for all populations, resulting in reduced HIV incidence and improved health outcomes, reduced morbidity and mortality among persons living with HIV (PLHIV) including those with tuberculosis (TB), and elimination of mother to child transmission (eMTCT) of HIV.
