This HRSA grant is for the purpose of optimizing the health, quality of life, and well-being of children with medical complexity (CMC) and their families. The initiative achieves this by establishing a coordinating center designed to support up to five associated demonstration projects. These projects are focused on implementing and evaluating evidence-informed, patient/family-centered models of care delivery for CMC. The coordinating center will provide essential assistance to these demonstration projects in meeting their program objectives, facilitate the development and widespread dissemination of valuable resources, and offer leadership to the broader field. Ultimately, the grant seeks to enhance overall systems of care for this vulnerable population.
