Description: | The purpose of this supplemental announcement is to provide funding to assist CPPW awardees in expanding the impact of the initiative to by creating local and national capacity to provide peer to peer mentoring, consultative support and training as well as sharing and dissemination of lessons learned about local processes, implementation and outcomes in order to increase local and national capacity to implement policy, systems, and environmental change approaches to prevent and reduce chronic disease. Peer to Peer Mentoring is designed to provide structured opportunities for ARRA/CPPW grantees, to share their knowledge and experience applying the CPPW interventions and strategies by working with other communities that have experience and demonstrated success in policy and environmental change. Demonstrated success may be outside of the MAPPS strategies being recommended in the CPPW funded communities. Mentors are expected to be experts in applying evidence and practice-based MAPPS (Media, Access, Point of decision information, Price and, Social support services) strategies to the specific needs of mentored communities and addressing health disparities. Ideal candidates must also have experience in and demonstrated capacity to facilitate training and consultative support. Recipients will be required to participate as a member of the national CPPW Steering Committee group. Applicants for this program will have selected communities that meet the following criteria. : |
