Additional Information on Eligibility: Eligible applicants are public or private non-profit organizations with substantial demonstrated successful experience and expertise in providing leadership at the national level to state units on aging, state public health departments, area agencies on aging, and other local aging, disability, public health and community-based organizations related to the implementation and sustaining of chronic disease self-management education programs and other evidence-based programs targeted to older adults and adults with disabilities. Individuals, foreign entities, and sole proprietorship organizations are not eligible to compete for, or receive, awards made under this announcement.

Description: The Administration on Aging (AoA), an agency of the Administration for Community Living (ACL) plans to award one cooperative agreement to support a National Resource Center on Chronic Disease Self-Management Education Programs (Center). The funding, from the 2013 Prevention and Public Health Funds (PPHF-2013), will support up to $790,000 for year 1 and up to $1 million each for years 2 and 3 of a three (3) year project period, contingent upon the continued availability of federal funds.
The Center will assist states, the aging, disability and public health networks and their partners to increase access to and sustain evidence-based prevention programs, particularly Chronic Disease Self-Management Education (CDSME) programs, that improve the health and quality of life of older adults and adults with disabilities.
The Center will serve as a national clearinghouse of tools and information on CDSMEs. The award will be made in the form of a cooperative agreement because AoA will be substantially involved.
This modified Funding Opportunity Announcement includes an increased funding amount for year 1 (increased from $730,000 to $790,000), as noted in the Executive Summary on page 1, in the Section I Description on page 2 and in the Section II Award Information on page 5.
