The purpose of this Amendment No. 02 to the RFA is to revise and restate the RFA document in its entirety. All changes from the RFA originally issued on April 20th, 2011 are indicated in red text. Please note the following:

1. The RFA No. has been corrected to indicate the full “Funding Opportunity No.” used for the www.grants.gov web site.
2. This Amendment reflects changes previously made by Amendment No. 01 to 1) extended the due date for receipt of written questions; and 2) extended the RFA closing date.
3. Please note that this Amendment clarifies the requirements for illustrative versus mandatory monitoring and evaluation indicators and therefore supersedes the information presented verbally at the Pre-Award Conference on May 10th, 2011.
