Amendment II: Application Due Date Extension and HHS Checklist Instructions for International Applicants

The purpose of this amendment is to provide further instructions for international applicants. There is a system error with the HHS Checklist (08-2007) [V4.0] form of the application package. There is a mandatory field in the HHS Checklist in Part C where an applicant must enter a U.S. state and zip code. The system will not allow an international entry for these fields, and entry is required to submit the application successfully. International applicants should select the state of Georgia (GA) and include the following zip code: 44444-4444.

This amendment also extends the Application Due Date from March 12, 2021 (3/12/2021) to March 22, 2021 (3/22/2021).
