Additional Information on Eligibility: a) Research proposed under this funding opportunity announcement may not overlap with research activities simultaneously funded under Optional Objective A: Inpatient MAARI VE Evaluation of FOA IP11-003: Annual Estimates of Influenza Vaccine Effectiveness for Preventing Laboratory-Confirmed Influenza in the United States and should so state in the applicaiton. b) Applicant institutions or applicant institutions that propose contract services with institutions or organizations that do not currently have a high frequency of clinical testing (goal: >65% of admitted patients with symptoms consistent with ARI) are eligible to apply but must include the following detail in the application: A plan to identify, enroll, and collect a respiratory specimen on patients with ARI within 48-72 hours of admission; Participation levels of >50% in previous studies enrolling >150 hospitalized adult patients; Ability to enroll the sample size(s) identified in Part 2. Section I.2.Approach (after accounting for participation). c) Eligibility for Optional Component funding will be limited to those applications recommended for funding of the Core Component. To be considered for Optional Component funding, the Optional Component application must have an overall priority score between 10 and 50.

Because influenza viruses are constantly changing and vaccines are reformulated every year, annual estimates of the effectiveness of influenza vaccines in preventing influenza infection and its associated complications are needed in order to evaluate the protection provided by annual, nationwide vaccination programs.  The goal of this funding opportunity announcement is to establish a sustainable network of US institutions that can obtain hospitalization data and provide accurate estimates of annual influenza Vaccine Effectiveness (VE) so as to mitigate and prevent influenza-associated hospitalizations in US adult populations. Participating institutions will coordinate enrollment of adult hospitalized patients with influenza infection, confirm influenza infection status using sensitive laboratory-based molecular assays, and estimate VE using a test-negative study design.  Annual VE estimates will be used to inform and assess the public health impact of influenza vaccination programs on the prevention of health-related complications due to influenza infection, especially in older adults.
