The Administration on Aging (AoA), a division of the Administration for Community Living (ACL), is offering a cooperative agreement grant to establish a National Chronic Disease Self-Management Education Resource Center. This grant is for two primary purposes. Firstly, it aims to significantly increase the participation of older adults and individuals with disabilities in evidence-based Chronic Disease Self-Management Education (CDSME) and self-management support programs.
