The Administration on Aging (AoA), an agency of the Administration for Community Living (ACL), is offering this grant to support a National Resource Center on Chronic Disease Self-Management Education Programs (Center). This grant is for establishing a national clearinghouse of tools and information on evidence-based prevention programs, particularly Chronic Disease Self-Management Education (CDSME) programs. The Center will assist states, aging, disability, and public health networks, and their partners to increase access to and sustain these programs, ultimately improving the health and quality of life for older adults and adults with disabilities. Funded by PPHF-2013, the cooperative agreement provides up to $790,000 for year one and $1 million for years two and three of the three-year project.
