Additional Information on Eligibility: | An entity may apply for only one Option and must clearly identify this Option in their application. Eligible applicants for the Option 1 state grants are domestic public or private non-profit entities including state and local governments, Indian tribal governments and organizations (American Indian/Alaskan Native/Native American), faith-based organizations, community-based organizations, hospitals, and institutions of higher education. Federally Recognized Tribes and Tribal organizations representing Federally Recognized Tribes are eligible to apply for the Option 2 tribal grants. Individuals, foreign entities, and sole proprietorship organizations are not eligible to compete for, or receive, awards made under this announcement. |

Description: | Through this funding opportunity, the Administration on Aging (AoA), part of the Administration for Community Living (ACL), plans to award two different types of falls prevention grants financed solely by 2014 Prevention and Public Health Funds (PPHF-2014). The Option 1 state grant awards will support approximately 8-10 grants to domestic public or private non-profit entities averaging about $400,000 per award, for a two-year forward funded project period. The Option 2 tribal grant awards will support approximately five grants to Federally Recognized Tribes and Tribal Organizations up to $100,000 per award for a two-year forward funded project period. An entity may apply for only one Option and must clearly identify this Option in their application.
This funding opportunity is designed to increase the number of older adults, American Indian, Alaskan Native and Native Hawaiian elders, and adults with disabilities at risk for falls who participate in evidence-based community programs to reduce falls and falls risk, while concurrently increasing the sustainability of these programs through innovative financing arrangements. The awards will be made in the form of cooperative agreements because AoA will be substantially involved. These cooperative agreements have an anticipated start date of September 1, 2014. |
