The goal of this NOFO is to strengthen the capacity of the Government of Haiti (GOH) to improve health outcomes and reduce mortality and morbidity through the routine use of patient and health services data. The NOFO seeks to facilitate this goal by reengineering the health information system (HIS) and i) increasing its use as a monitoring tool for continuity and quality of care, ii) to strengthen public health surveillance for early detection of outbreaks, and iii) to improve the capacity to respond to complex health emergencies. Lastly, this NOFO will improve the GOH’s ability to monitor health outcomes including natality and mortality by improving the National Vital Registry system.

Version History

Version Modification Description Updated Date
Amendment III: Increase to the Approximate Total Fiscal Year Funding for Year 1
The purpose of this amendment is to increase the Approximate Total Fiscal Year Funding for Year 1 from $4,000,000 to $6,500,000.

This amendment is for informational purposes only. CDC is not accepting new applications for consideration.

Sep 13, 2021
Amendment II: Questions and Answers (Q&A) and Additional Updates
The purpose of this amendment is to include the updates as listed below.

•This amendment includes questions and answers (Q&A) in Section H. Other Information.
•Any applicant who downloaded the application package prior to March 24, 2021 (3/24/2021) must restart their application package due to a mandatory update to the HHS Checklist in the required application package forms in Grants.gov. Restarting the application process is required because the HHS Checklist will be removed from the application package on March 24, 2021. NOTE: Once the applicant logs into Workspace, there will be a warning informing them that the application forms have changed for the NOFO they are applying for.
•There is a change to the supporting documentation requirements under Criteria (2) Entity other than a sole proprietorship, eligibility area (2) of the Phase III PEPFAR Local Partner Funding Preference on pages 47-49.
•Applicants should also note that implementation and enforcement of the PLGHA Standard Provision is waived effective immediately. This change is due to the Presidential Memorandum, “Memorandum on Protecting Women’s Health at Home and Abroad,” signed on January 28, 2021, which revoked the January 23, 2017, Presidential Memorandum implementing the PLGHA policy, and thereby rescinded the PLGHA Policy. Accordingly, CDC confirms that the Funding Restriction referring to the United States Protecting Life in Global Health Assistance policy, and the Administrative and National Policy Requirement AR-35: Protecting Life in Global Health Assistance, which were included in the original publication, have been removed and are no longer applicable to this NOFO.

Mar 01, 2021
Due to an update in our application packages on Grants.gov, if you retrieved a package before today March 24, 2021, you will have to retrieve an updated application package with the new versions of the updated forms.
If you have any questions please contact ogstims@cdc.gov.
