Amendment II: Application Due Date Extension and HHS Checklist Instructions for International Applicants

The purpose of this amendment is to provide further instructions for international applicants. There is a system error with the HHS Checklist (08-2007) [V4.0] form of the application package. There is a mandatory field in the HHS Checklist in Part C where an applicant must enter a U.S. state and zip code.
