Additional Information on Eligibility:: | The intent of this FOA is to engage with a wide variety of interested parties who have developed innovations that will drive significant improvement in three-part aim outcomes. The target applicants for this solicitation are providers (e.g., specific service providers, clinician groups and/or hospitals); States applying in partnership with providers; managed care organizations (MCOs) applying in partnership with providers; and conveners applying in partnership with providers. All applicants not directly providing prenatal care services (i.e. States, MCOs, and conveners) are expected to partner with providers of obstetrical care services and proposed enhanced prenatal care services. Included within the definition of State, for the purposes of this FOA are the District of Columbia and the 5 U.S. Territories: American Samoa, Guam, Northern Mariana Islands, Puerto Rico, and the U.S. Virgin Islands. Faith-based organizations, Community-based organizations, and Tribal organizations are also eligible to apply. |

Synopsis 2 | Currently, applications are due on June 13, 2012. However, since CMS announced the call for applications, stakeholders have raised a number of important issues, including challenges with obtaining state data and conducting the necessary linkages. In order to address these issues, CMS is revising the FOA and providing further clarifications regarding the initiative. We are extending the application deadline for the Strong Start for Mothers and Newborns models TEMPORARILY to July 13, 2012. Please DO NOT submit your application until the official amended FOA and guidance have been released. If you submit your application prior to the release of the amended FOA, which will include revised application and program requirements, you will not be able to revise a previously submitted application in response to the newly available information. Those who submit their applications prior to the release of the amended FOA will be required to submit a new application in order to be eligible for award under the revised requirements in the amended FOA. As such, we will individually contact each applicant who has already submitted an application within the next week with further information. We will provide a final deadline in the revised FOA. Those who did not submit a Letter of Intent by the May 11, 2012 deadline may submit an application for Strong Start because we will no longer be requiring an LOI.
