The Community Health Aide Program (CHAP) Office is offering this grant to support Tribes and Tribal Organizations (T/TO) in planning and implementing Community Health Aide Programs across the contiguous 48 states. This grant is for fostering regional flexibility, enabling T/TOs to develop CHAP models tailored to their communities’ unique healthcare needs. Key focus areas include establishing clinical supervisor support for primary care, behavioral health, and dental disciplines, identifying essential technology infrastructure, and developing comprehensive training plans for prospective health aides. The program also aims to seamlessly integrate CHAP workforces into existing Tribal health systems, address social determinants affecting recruitment and retention, and estimate the total costs of full implementation, ultimately strengthening Tribal healthcare delivery.
