Community Selection application: Project application: For a dedicated HMIS grant, the application must either be from the UFA (when there is a UFA for the community) or the HMIS Lead (when there is no UFA for the community) that will be the grant recipient. To be considered for funding, Project Applicants must complete the information required by HUD, receive the approval of the YAB for their project(s) through a letter of support, and receive the approval of the CoC to apply for funding, as signed off on by the Collaborative Applicant or its designee. This NOFO application is for community selection only. Applications for funding individual projects will occur after communities have been selected. Requirements for applying for projects are detailed in Appendix A. As required in the Code of Federal Regulations (CFR) at 2 CFR 25.200 and 24 CFR Part 5 Subpart K, all applicants for financial assistance must have an active Data Universal Numbering System (DUNS) number (http://fedgov.dnb.com/webform) and have an active registration in the System for Award Management (SAM) (www.sam.gov) before submitting an application. Getting a DUNS number and completing SAM registration can take up to four weeks. Therefore, applicants should start this process or check their status early. See Section IV.C for information on SAM and the DUNS number requirement. See also Section IV.B for necessary form and content information. Individuals, foreign entities, and sole proprietorship organizations are not eligible to compete for, or receive, awards made under this announcement. |

[Section I.A.1] Only CoC Collaborative Applicants may apply to this NOFO [Section III.A] Applications are submitted through grants.gov, and are due by July 27, 2021 Communities represented by the CoC Collaborative Applicant must include a youth action board, the local or state public child welfare agency, and a broad array of other partners [V.A] The rating and ranking criteria included in this NOFO will be used to competitively select the communities [Section V.A] The selection of the selected communities will be announced in September 2021 Selected Communities will: Develop and implement a CCP to prevent and end youth homelessness [Section III.F.b] NOFO Priority The purpose of the YHDP is to implement projects that demonstrate how a comprehensive approach to serving homeless youth age 24 and under can dramatically reduce youth homelessness. The population to be served by this demonstration program is youth experiencing homelessness, including unaccompanied and pregnant or parenting youth. In order to effectively implement a system that addresses the needs of youth experiencing homelessness, Continuums of Care (CoCs) must understand the subgroups of unaccompanied youth and incorporate those understandings into the YHDP CCP and awarded projects. This will require CoCs to use innovative practices to design better projects and strong comprehensive plans to prevent and end youth homelessness. |
