Question: How do applicants submit the required ED-80-0013 and Title VI Assurances forms? Answer: Grantees will need to print out and sign: (1) ED-80-0013, and (2) Title VI Assurances. These forms, with signatures, will then be submitted as attachments. The forms may be found as in the FOA Package – on pages 31 and 32. Question: In FY15 applicants were required to submit programmatic certifications for planning and training grants. Are these required for the FY16 application? Answer: No. Due to the statutory changes in the Fast Act, the programmatic certification forms are no longer used.
