The Cooperatives will engage key stakeholders, including Medicaid agencies and managed care organizations, to identify and address implementation barriers for evidence-based care delivery improvements. Furthermore, they will build the capacity of healthcare policy, payment, community, and delivery organizations to effectively implement PCOR evidence and foster continuous learning. The initiative also requires the evaluation of Cooperative activities and the establishment of a robust support structure to ensure all efforts are integrated and aligned, ultimately accelerating the translation of actionable knowledge into practice for improved patient outcomes.
