Additional Information on Eligibility: 1. Eligible Applicants:
Applicants must meet one of the following two criteria:
-Morbidity Criteria: Metropolitan Statistical Areas/Divisions with >2,000 Black and/or Hispanic MSM living with diagnosed HIV
o Focuses on MSA targets areas where most MSM are accessible
o Threshold of 2,000 ensures a large participant pool to robustly demonstrate the prevention and care service models
-Mortality Criteria: States in the 4th quartile for mortality rates as reported in the State Progress Report and have >1,000 Black and/or Hispanic MSM living with diagnosed HIV in a specified MSA
Jurisdictions eligible under the morbidity criteria cannot apply under the mortality criteria.
State health departments with one eligible MSA are required to work in the specified eligible MSA or MD. States that have more than one eligible MSA may elect to work in all or a reduced number of eligible MSAs, in order of prevalence unless one MSA/MD declines to participate.
States that have local health departments directly funded by CDC are not eligible to apply for the MSAs or MDs that contain locally funded health departments within the state.
Funding will be awarded to up to 2 health department that meets the Mortality Criteria.
CDC will not consider any application that is submitted: after the due date of June 1, 2015, 11:59 p.m. U.S. Eastern Standard Time; incomplete; missing descriptions of any of the required 24 services for comprehensive HIV prevention and care (Strategies and Activities section); or missing MOAs, MOUs, or letters of support for service agreements from each collaborative partner except the CBO partner(s). Purpose: To decrease the burden of HIV among MSM of color, this FOA supports health departments to collaborate with CBOs clinics and/or health care providers to implement models of comprehensive HIV prevention and care services for MSM of color who are at substantial risk for or living with HIV infection. Health departments and organizations within the collaboratives will prioritize services for this population, but no persons will be turned away, regardless of their race, ethnicity, or other demographic characteristic. Services may also be provided for persons besides MSM of color who are at risk for HIV acquisition and PLWH who are not virally suppressed.
