Eligibility

Eligible Applicants:: Native American tribal organizations (other than Federally recognized tribal governments)
Additional Information on Eligibility:: This competition is open to new organizations proposing to replace the current grantee, and applicants for open service areas as described in Appendix B. New organizations must demonstrate that they will serve the existing patients, populations, scope of services and service areas currently served by the grantee they intend to replace. Applicants must identify the grantee they intend to replace. Eligible applicants include public and nonprofit private entities. Faith-based and community-based organizations, Tribes, and tribal organizations are eligible to apply.

New applicants intending to replace a current grantee must be public or private nonprofit entities that are:

a) Federally-qualified health centers under section 1905(1)(2)(B) of the Social Security Act;
b) Grantees under section 1001 (regarding family planning) other than States;
c) Comprehensive hemophilia diagnostic and treatment centers;
d) Rural health clinics;
e) Health facilities operated by or pursuant to a contract with the Indian Health Service;
f) Community-based organizations, clinics, hospitals and other health facilities that provide early intervention services to those persons infected with HIV/AIDS through intravenous drug use; or
g) Nonprofit private entities that provide comprehensive primary care services to populations at risk of HIV/AIDS, including faith-based and community-based organizations.

Description:: This competition is open to new organizations proposing to replace the current grantee, and applicants for open service areas as described in Appendix B. New organizations must demonstrate that they will serve the existing patients, populations, scope of services and service areas currently served by the grantee they intend to replace. Applicants must identify the grantee they intend to replace.
The purpose of the Ryan White HIV/AIDS Part C Program is to provide HIV primary care in the outpatient setting. Applicants must propose to provide a comprehensive continuum of outpatient HIV primary care services in the targeted area including: 1) HIV counseling, testing, and referral; 2) medical evaluation and clinical care; 3) other primary care services; and 4) referrals to other health services. Primary care for persons with HIV disease should start as early in the course of the infection as possible. However, entry into a Part C EIS program may take place at any point in the spectrum of the disease or the patient’s lifespan.
