the applicant was funded under any of the FY 2014, FY 2015, and/or FY 2016 FSS NOFAs, or
the applicant received an FSS program through a transfer or consolidation from a PHA that was funded under such NOFAs.

Appendix D provides a list of applicants who meet the criteria set forth in the bullet points above. If you are not listed in Appendix D, but you believe you meet the criteria set forth in the bullet points above, you may request a review of eligibility from HUD by following the steps described in Section III.A.2 below.
HUD does not award grants to individuals. HUD will also not evaluate applications from ineligible applicants.
As required in the Code of Federal Regulations (CFR) at 2 CFR 25.200 and 24 CFR Part 5 Subpart K, all applicants for financial assistance must have an active Data Universal Numbering System (DUNS) number (http://fedgov.dnb.com/webform) and have an active registration in the System for Award Management (SAM) (www.sam.gov) before submitting an application. Getting a DUNS number and completing SAM registration can take up to four weeks; therefore, applicants should start this process or check their status.

2. Request for Review of Eligibility. In advance of submitting this grant application, applicants who are not listed in Appendix D but who believe they meet the eligibility criteria in the Section above (Section III.A.1 of this NOFA) must submit a Request for Review of Eligibility via email to fss@hud.gov. The subject line should be "2017 Review of FSS Eligibility." The email must include the applicant’s name, PHA number (in the format found in Appendix D), and a brief explanation of why the applicant believes they are eligible. HUD will review and respond within five working days of receipt of the request. That response will be an eligibility approval, eligibility disapproval, or a request for more information. If more information is requested, HUD will respond within five working days upon receipt of the additional information. All initial Requests for Review of Eligibility must be received no later than 20 calendar days prior to the due date of this NOFA. Individuals, foreign entities, and sole proprietorship organizations are not eligible to compete for, or receive, awards made under this announcement.
