This grant, offered by HUD, is for the purpose of funding Service Coordinator positions within Multifamily Housing (SCMF) to support elderly individuals and non-elderly persons with disabilities residing in HUD-assisted properties. Service Coordinators play a vital role in connecting residents with essential community-based supportive services, promoting independent living, and preventing unnecessary transitions to higher levels of care. They facilitate access to resources, enhance financial security, foster social connections, and improve overall health and well-being. Services are voluntary and tailored, including support for nutrition, housekeeping, healthcare coordination, public benefits, and financial management, aiding Activities of Daily Living. Coordinators also provide educational programming and help property management understand resident needs, ensuring residents can age in place with dignity and support.
