Description: This initiative will support innovative, collaborative, and multi-disciplinary research designed to study the effective adaptation, integration, and implementation of recommended guidelines of care of persons with multiple chronic conditions (MCCs) from populations that experience health disparities. Projects would be expected to involve more than one component and/or more than one level of influence within existing or newly proposed health care models. The ultimate goal of this initiative is attainment of optimal treatment and health outcomes goals in order to move towards health equity.
