Eligible Applicants:: Others (see text field entitled “Additional Information on Eligibility” for clarification)
Additional Information on Eligibility:: To qualify for funding, an applicant must be a Tribal Epidemiology Center currently supported by the Indian Health Service or an Urban Indian Health Program supported by IHS, under Title V of the Indian Health Care Improvement Act, PL 94-437, as amended.

The organization submitting the application will:

Serve as the lead agency for the project, responsible for its implementation and management; and

Serve as the fiscal agent for the Federal grant awarded.

OMH encourages TECs and UIHPs to work collaboratively on this project. Applications from a group or consortium must identify one of its members as the lead agency for the project.

To demonstrate coordination between the TEC and/or UIHP, and participating Tribes, letters of support and collaboration from the participating Tribes should be included with the application.

Description:: The purpose of the American Indian/Alaska Native Health Disparities Grant Program is to improve the effectiveness of efforts to eliminate health disparities for American Indian and Alaska Native communities. Through this FY 2012 announcement, the OMH is supporting utilization of epidemiological data to identify high priority health status objectives and to make recommendations relative to the services and/or activities required to address those objectives; development of alliances and partnerships to improve coordination of and access to quality health services; and development of programs designed to increase the representation of the American Indians/Alaska Natives in the health care workforce (including research positions).
