Additional Information on Eligibility: | Applicants must be physically located and operate in the state or territory for which work is proposed. Applicants must specifically identify the state or territory in which they will work. Applicants with locations in multiple states or territories must select one state or territory, the state or territory in which that applicant is located. Applicants proposing to work in states or territories other than where the applicant is physically located and operates will be considered non-responsive and will not receive further review.CDC will not accept and review applications with budgets greater than the ceiling of the award range. Applications with budgets that exceed the ceiling of the award, which includes both direct and indirect costs, will be considered non-responsive, and will not be entered into the review process. |

The Department of Health and Human Services (HHS), Centers for Disease Control and Prevention (CDC), and National Center for Chronic Disease Prevention and Health Promotion (NCCDPHP) are accepting applications for Fiscal Year (FY) 2024 The Behavioral Risk Factor Surveillance System (BRFSS): Impact on Population Health.The BRFSS is the nation’s premier system of a health-related telephone survey that collects state data about health-related risk behaviors, chronic health conditions, and the use of preventive services by U.S. residents. Since its establishment in 1984, BRFSS has grown to be the only continuous, state-based health surveillance system that is conducted nationwide. The survey is divided into Component A – Core and Component B – Asthma Call Back Survey (ACBS).The purpose of this multi-component Notice of Funding Opportunity (NOFO) is to provide financial and technical assistance to government and non-government applicants to maintain the surveillance system and expand the use of BRFSS data to inform public health actions to improve health. The objectives are as follows:Conduct population-based surveillance of behavioral risk factors, chronic health conditions, and the use of preventive services among the U.S. adult population.Analyze, disseminate, and use, BRFSS data to assess trends, direct, evaluate, and establish program priorities, inform policy development, and target relevant population groups.Provide real-time BRFSS data to support decision-making for interventions and programmatic activities that drive public health actions (PHAs) that demonstrate public health impact. |
