Description: The purpose of this supplement is to support a state health department to evaluate the implementation of the STEADI fall prevention toolkit within a large healthcare organization with a focus on health outcomes and cost.
State health departments funded for the Core VIPP State Falls Prevention Program component are currently implementing fall prevention activities and programs in their state. As part of this work, the states are implementing 3 community-based programs (Tai Chi Moving for Better Balance, Stepping On, and Otago) along with the clinically focused STEADI toolkit—a set of tools that facilitates older adult fall risk screening, assessment, treatment, and management in primary care settings. To support implementation of STEADI, state health departments have developed partnerships with large health systems. These partnerships have presented the opportunity to conduct a local evaluation of the implementation of STEADI within a large healthcare organization to understand the potential health outcomes and costs associated with STEADI implementation.
Specifically, supplemental funding would support a state health department to evaluate a local implementation of STEADI in a large healthcare organization or organizations, including collection of process, health outcomes, and cost data.
