This shall include contracts and/or funding agreements complete list of the contracts and/or funding agreement under which the applicant has provided the above quitline services for within the United States, including: (A) The beginning and ending dates of the contracts/funding agreements; (B) The services provided under those contracts/funding agreements;(C) The name, address, and telephone number of at least one manager in each client organization who is personally familiar with the applicant’s performance under the contract/funding agreement. |
