This grant is for the purpose of strengthening health information technology (health IT) infrastructure and exchange capabilities within communities. The Beacon Community Cooperative Agreement Program aims to empower hospitals, clinicians, and patients to become meaningful users of health IT, leading to measurable improvements in healthcare quality, safety, efficiency, and overall population health. Funding is provided through cooperative agreements to approximately two qualified non-profit organizations or government entities representing geographic healthcare communities. Ideal candidates are national leaders in health IT, workflow redesign, care coordination, and quality monitoring, demonstrating advanced rates of electronic health record (EHR) adoption and health information exchange (HIE). Recipients will enhance existing competencies over a 31-month period, generating valuable lessons for nationwide healthcare improvement.
